**Job Description: Physical Therapist**

**Patient Care:**
Helps patients with physical problems, including those with wound care. Creates care plans that follow all rules and best practices.

**Assessments:**
Uses standard tests and proven methods to help with patient care.

**Treatment Procedures:
** Performs various treatments as part of the rehabilitation plan. This includes hands-on techniques, helping patients walk, exercises, using different tools, and caring for wounds.

**Progress Check:**
Looks at how well patients are doing and changes treatment plans if needed. Records this information according to rules.

**Outcome Evaluation:
** Reviews how effective the treatments are and changes services based on patient progress. Refers patients to other services when necessary.

**Consultation:
** Offers advice and support to patients and their families about physical problems.

**Communication:**
Shares updates about patient progress and treatment plans with supervisors and other healthcare team members.

**Discharge Summary:
** Writes a complete summary of care provided when patients leave, following required guidelines.
- **Documentation:
** Keeps accurate records in the electronic system and ensures patient files are maintained according to company policies.

**Recording Treatment Time:
** Records how long treatments take, following all necessary guidelines.

**Meetings:** Participates in important meetings, such as patient care discussions and reviews.

**Training:**
Takes part in required training sessions and company conferences.

**Supplies:** Able to get necessary equipment or supplies according to rules.

**Supervision:** Can guide and help physical therapist assistants, rehabilitation aides, and students.
- **Professional Standards:** Follows the guidelines and ethics required for practicing as a Physical Therapist.
**Qualifications:**
- Must have 1 year of experience as a Physical Therapist.
- Experience in long-term care (LTC) or rehabilitation is required.
- Must hold a Missouri State License.
- New graduates are welcome if they have completed a Level II experience, not just Level I observation.
**State License Requirements:**
- Missouri - Compact License is allowed.
