We are looking for a friendly and dedicated Clinical Pharmacist to join our dynamic team in a community pharmacy that strives to make a positive impact on our customers’ quality of life. Please note this is a remote position for a 4-month contract to cover a maternity leave. This pharmacist position will primarily focus on two roles. - Medication reconciliation review for patients being admitted into long-term care beds or complex continuing care beds.
- Completing 3-month and annual medication reviews for residents of our long-term care facilities.
Applicants must possess: • A degree in Pharmacy and a license in Ontario, and be in good standing with the Ontario College of Pharmacists
• Geriatric Pharmacy Specialty Certification (BCGP) or PharmD with at least 2 years LTC experience.
• Excellent communication skills in English; additionally, French would be an asset.
• Exceptional customer service skills, as well as have a collaborative approach to working with all other pharmacy team staff, and healthcare professionals
• A strong desire to deliver excellent patient care in an empathetic and warm manner: providing answers to questions pertinent to common medical needs (i.e., diabetes, asthma, smoking cessation, high blood pressure), counsel and train patients on proper use of medical devices and supplies, and partner with healthcare professionals to review the quality and effectiveness of drugs, providing advice on usage and observed side effects.
• An ability to work efficiently and with a high degree of accuracy
• An understanding and interest in providing services under the expanded scope of practice
• Experience in providing clinical knowledge and dispensing compliance packaged medication to Nursing Homes and LTC is a requirement
•Experience using Kroll, Point Click Care, and E-Mar is an asset
Job Type: Temporary Part-time (2-3days per week) Work Period: May 26 - September 30, 2025
